I haven't actually taken the modchip plunge on my PS2 yet (because the hardware is so damn flaky that I'd rather have the free repairs for DRE problems), but what I hear is that the media is very important.
A lot of people swear by Ritek media, and I've heard Verbatim mentioned as well. However, like I said, I don't know how accurate that is as I have zero experience actually doing this.
Step 2, once you have the media, is to burn, at as slow a speed as possible, the image to disc using DVD-RW software that can read whatever image format you have. Just about everything reads ISO and BIN/CUE, but sometimes you get .ccd, .mdf, .cdr, and some other formats. Regardless, look up the file extension for software that supports it.
After that, pop it in and hope for the best. As has been stated, you'll need some means of getting around the console's copy-prevention mechanisms, but at one point the DMS3 and Messiah chips were good choices. That was a few years ago so there are probably better ones out there.
You can try gamecopyworld's forums for more help, or there are plenty of other folks here who will be able to give you much more detailed info than I can.
Jam it back in, in the dark.
