One of my favorite deaths are killing sprees in UT2004 or Gunz: The Duel, when you rack up 4-5 kills in a row and then get the anger and wrath of other players on the chatline, as they wait to respawn.
Oh, and for NPC kills, I'd say a key moment in Half Life 2 when you kill Combine Soldiers with various objects using the gravity gun, nothin' like slicing through flesh with a rusty blade or fire extinguisher. Jam it back in, in the dark.
