Simple Solution: Talk to her

Just tell her that you're running out of time to buy the tickets and that you guys need to decide what's going on before it's too late. If she doesn't want to go, tell her to let you know before the last day to buy tickets comes. If she wants to go, try to talk to each other and get out of the avoiding each other mode so that it's not awkward during prom.

If you're suggesting giving her a note, then no, that won't work. Why? Because she can refuse to answer it and stall. Talk to her in person and be straightforward (but don't be an ass).
