That really depends on the game. In counter-strike, there's a fair few servers out there who have admins which only come on for short periods or barely ever. Then you have a bunch of people without any authority, team killing and ruining it for others.
Then again, counter-strike is pretty old. The system of "everybody needs to vote in order for this player to be kicked" is horribly flawed, because the offenders can just retry their way back into the server. Do you know how hard it is, to get everybody to type "vote ___" in the console? Pretty damn hard, that's for sure.
I mean, it doesn't even ban the person temporarily. A kick. Then they rejoin. What's the point in that? And it's not just FPS games either. Games like Warcraft III or Starcraft can turn ugly as there's no kick option once the game has started.

See, I only play online on Xbox Live where it's generally very easy to kick someone out for team killing. I can't think of any games that ask for more than three votes.
