Twisted Metal Black for PS2.
Having been a huge fan of Twisted Metal 2, I got the game as a gift, and it completely shocked me with its horror-theme. There are plenty of unforgettable (in a good and bad way) moments in its excellent CG sequences, and I'll bet anyone who's played it would agree. The game has since become one of my favorites, and no game has scared me since. Hopefully they make a Twisted Metal Black 2 for PS3. Jam it back in, in the dark.
