I've had a similar situation to this occur in the past.
My parents computer had a monitor with built in powered USB ports. When the monitor would go into sleep, or standby, with the USB ports would cause some sort of disconnect. Once I disconnected the main USB line from the PC to the monitor the problem went away. You could listen for a usb disconnect chime when the monitor goes to standby/sleep mode.
