In the cases you mentioned, for instance, no lettuce, it comes with it, I'll not ask for another one, but will politely remark with the manager the fact, expliciting that I do not want a refund or a new whatever is it, just wish they are more carefull with the orders.
But in the other hand, if I ask for something, and they deliver me something else, I'll complain, and in certain cases, I'll ask for a refund instead, because I'm easily grossed out, and can't stand the fact that the cook could've made something funny with the food.
