You could try StyleXP which works in the same way as a patched uxtheme.dll, and I believe you can use any Windows Visual Styles with StyleXP. The other popular program is WindowBlinds, which certainly more powerful than Visual Styles/StyleXP but it is an extra process at startup. (Which I don't like, so I just use Visual Styles.) It is a part of the ObjectDesktop by StarDock, who make many other applications which change the way Windows look.

One of the easiest ways to replace all the icons is IconPackager, again a product of StarDock and a part of ObjectDesktop. Most of the popular icons packages are converted to IconPackager format.

Jam it back in, in the dark.

You're correct, all of these are extra processes which use up resources; I only use IconPackager because I do not know of a way to easily change all the icons without using an external application. (Of course, if you do know, please point me in the right direction.) Although, applications such as WindowBlinds are much more powerful than just plain Visual Styles.
