The most recent incident for me happened because I hadn't paid attention to my tires for a while. A dog ran out in front of me on the freeway, I braked hard, and started spinning out.
I ended up with my car across the leftmost lane facing the median. Luckily, most people behind me had seen it and were slowing down or avoiding me - how ever, this motorcycle did not until it was nearly too late, and nearly hit me before I righted myself and drove off.
